HIPAA Policy
Teaching Tiny Teeth

This Notice describes how medical and dental information about your child may be used and disclosed, and how you can access this information.
Please review it carefully.
1. Our Responsibilities
Teaching Tiny Teeth is committed to protecting the privacy of your child’s health information. We are required by law to:
- Maintain the privacy of Protected Health Information (PHI)
- Provide you with this Notice of our legal duties and privacy practices
- Follow the terms of this Notice currently in effect
- Notify you if a breach occurs that may have compromised your child’s PHI
2. How We May Use and Disclose Health Information

To provide dental screenings, cleanings, fluoride treatments, sealants, X-rays, exams by collaborating dentists, referrals, and follow-up care.
b. Payment
To bill Medicaid, CHP+, insurance companies, or other responsible third parties for services provided.
c. Healthcare Operations
For quality improvement, compliance, internal audits, credentialing, and training purposes.
d. Required or Permitted by Law
We may disclose PHI when required by federal or state law, including:
- Public health reporting
- Abuse or neglect reporting
- School health requirements
- Law enforcement requests
- Health oversight agencies
- Judicial or administrative proceedings
e. To Parents, Guardians, and School Officials
We may share relevant information with:
- Parents/legal guardians
- School nurses
- School administration (when needed for safety or coordination)
f. Emergency Situations
If a child is in danger or requires emergency care, we may share the minimum necessary information to protect their health.
3. Uses and Disclosures Requiring Written Authorization
We will obtain written authorization from you before using or disclosing PHI for any purpose not listed above, including:
- Marketing
- Sharing information with outside providers NOT involved in your child’s care
- Release of records to any third party not permitted by law
You may revoke your authorization at any time in writing.
4. Your Rights
As a parent or legal guardian, you have the right to:
a. Access
Request to see or receive a copy of your child’s dental records, X-rays, and visit summaries.
b. Amend
Request corrections to your child’s health information if you believe it is incorrect or incomplete.
c. Restrict Disclosures
Request limitations on how we use or disclose PHI.
(We are not required to agree to all requests, but we will consider them.)
d. Confidential Communications
Request that we contact you in a specific way (e.g., via email, phone, alternate number).
e. Accounting of Disclosures
Request a list of times we disclosed PHI for reasons other than treatment, payment, or operations.
f. Receive a Paper or Electronic Copy of This Notice
You may request a printed or emailed copy at any time.
g. File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with:
